Southport and Ormskirk Hospital NHS Trust provides healthcare in hospital and the community to 258,000 people across Southport, Formby and West Lancashire.
Acute care is provided at Southport and Formby District General Hospital and Ormskirk and District General Hospital. This includes adults’ and children’s accident and emergency services, intensive care and a range of medical and surgical specialities. Women’s and children’s services, including maternity, are provided at Ormskirk hospital.
The North West Regional Spinal Injuries Centre at Southport hospital provides specialist care for spinal patients from across the North West, North Wales and the Isle of Man.
The Trust also provides sexual health services for the Metropolitan Borough of Sefton.
The 18 week tracker/validator plays a crucial role within the Directorate, working closely with the Directorate Manager and Operational Service Manager you will ensure efficient and effective operational management for each of the Directorate Services and to support the Directorate’s strategic agenda in relation to performance targets.
The post holder will be responsible for the tracking and data validation of patients on the 18 week pathway within both Dermatology and ENT, however the post holder will be expected to provide cover for the other 18 week trackers and their specialities as required.
The role involves working collaboratively with key staff across the Trust including Medical Secretaries Access Teams and Clinicians to investigate 18 week breaches and amend pathways as appropriate on the electronic patient record in accordance with 18 week rules.
There will also be an element to education to key personnel in order to expand the knowledge base of the 18 week pathway leading to more efficient and effective patient flows.
The successful candidate will be able to demonstrate excellent interpersonal and communication skills and will have the ability to forge and maintain good working relationships with colleagues. The ability to manage a very busy workload is essential, as is the ability to quickly find solutions and to identify and focus on critical issues when decision making. Knowledge to the 18 week rule suite and patient pathway management is essential as is an understanding of the electronic patient record.
